Job Description:
As aClinical Case Manager II, you will provide support for the Medical Directors of the NAVIGATE Program at the UF Health Springhill Clinics. NAVIGATE is a comprehensive program designed to provide early and effective treatment for individuals experiencing a first episode of psychosis. The program offers Coordinated Specialty Care (CSC) for early psychosis, helping individuals and their families navigate the mental health system to achieve psychological and functional well-being.
Responsibilities include:
+ Case Management Services: Documents NAVIGATE team evaluations and management plans for patients. Works with the team to develop effective treatment plans and works with the patient on a day-to-day basis using professional judgment and discretion to implement the approved treatment plan. Coordinates direct patient therapy services as outlined in the NAVIGATE protocol. Ensure individual resiliency and family education therapy sessions are scheduled and provide the services if clinical coordinators are unavailable.
+ Program Support: Actively develops community relationships to enable direct assistance in services for NAVIGATE patients and families. Assists NAVIGATE patients in assessing housing, transportation, and other community resources. Ongoing patient outreach, support and follow-up with appropriate documentation as required by the program funding agency. Meet weekly with team members advocating for patient and caregiver needs. Compile patient lists for weekly case conference and perform necessary follow-up.
+ Community Outreach: Collaborate with community resources and agencies to connect families with services needed. Community Outreach: Provide support and assistance to individuals and their families diagnosed with first episode of non-affective psychosis (FEP). Provide collaborative consultation to schools and agencies serving individuals with FEP. Provide public education programs for community members to promote community awareness and acceptance of persons with FEP. Collaborate with community resources and agencies to connect families with services needed.
+ Program Reporting: Track, compile and develop protocols to provide reporting needs for the NAVIGATE program’s funding agency. Document and track consultations and crisis management activities for the reporting agency.

TheDepartment of Psychiatry (https://psychiatry.ufl.edu/) at the University of Florida are engaged in groundbreaking projects covering a variety of interests through our Centers, labs, and collaborations with other departments. The Department of Psychiatry includes a large outpatient center with various specialty clinics within (including one associated with our Center for Autism and Neurodevelopment), a very active inpatient service, psychiatry collaborative program for primary care practitioners, school consultation program, and community consultation services. The Department of Psychiatry spans services across adult, addiction, and child/adolescent specializations, including a freestanding psychiatric hospital, a large consult liaison service at the main UF Shands Hospital, the Florida Recovery Center operated by our Addiction Division, and multiple research centers including the Center for Addiction Research & Education and the Center for OCD and Related Disorders (COARD).
Expected Salary:$60,000 - $70,000; commensurate with education and experience.
Minimum Requirements:
Master’s degree in an appropriate area of specialization and two years of experience; or a bachelor’s degree in an appropriate area of specialization and five years of appropriate experience.
Preferred Qualifications:
+ Experience with schizophrenia and first episode psychosis
+ Experience with behavioral assessments and providing interventional therapy
+ One year of clinical consulting experience in helping individuals in psychiatric care programs
