In collaboration with the Cardiology leadership team and team leads, the Clinical Coordinator, Cardiology will support clinical care pathways for patients undergoing cardio-diagnostic procedures by enhancing patient experience, improving standards of care, and ensuring smooth transitions to appropriate services. To guarantee that the community has prompt access to care, prioritize patient referrals to the clinic and identify and close any gaps in training, protocols, equipment utilization, and best practices.

Reporting Relationship: Manager, Cardio Diagnostics and Respiratory Therapy
Position Responsibilities:
Conduct learning needs assessment in collaboration with clinical program leadership teams to address knowledge gaps in the delivery of cardio-diagnostics and device programs. Lead/oversee centralized intake model for cardiac device clinic.Monitor centralized processes, including prioritization of referrals to the carious cardiology program.Support in-patient clinical care pathways in improving patient experience, enhance patient care and coordinator smooth transition to care.Support in-patient through the reduction of length of stay and readmission rate. Evaluate new products, equipment, and procedures that impact the delivery, and quality, and cost of providing cardiovascular care to improve patient outcomes and resource management in collaboration with clinical program leadership.Lead the development, evaluation, and ongoing improvement of policies, models and standards of care, and clinical services for the assessment and management of patients undergoing cardio-diagnostic procedures.Conduct cardiac care and service delivery needs assessment and gap analysis to develop plans to improve clinical and non-clinical patient outcomes.Other duties as assigned.Qualifications:
Registered member of the College of Nurses of Ontario (CNO) in good standing with no restrictions to practiceCompletion of a Baccalaureate Degree in Nursing required Critical Care Certificate is requiredA minimum of 3 year’s Cardiology, Device Program (Pacemaker/ICD’s), Cardiac ICU, or related field experience requiredMembership with Nurses Association of Ontario (RNAO) preferredCoronary Care 1 & 2 is preferredCompletion of a Graduate Degree in Nursing, Education, or other related health discipline is an assetElectrophysiology Certificate is an assetExcellent decision making and problem-solving skills with ability to effectively prioritize multiple demandsDemonstrated commitment to professional practice standards and patient care needsProficiency in Microsoft OfficeExcellent attendance and discipline free record required
