Clinical Coordinator - Pharmacy

Job Overview:

The Pharmacy Clinical Coordinator is responsible for providing pharmaceutical care to patients in compliance with federal, state, and local regulations, as well as hospital policies. This role involves collaborating with pharmacy leadership and staff to ensure the safe, cost-effective, and appropriate use of medications. Key responsibilities include planning, implementing, evaluating, and monitoring pharmacy clinical programs, assisting in Drug Usage Evaluations (DUEs), and serving as an educator, mentor, and resource for pharmacy staff, students, and residents. Clear communication and the ability to prioritize projects to meet goals and deadlines are essential.

In addition, the inpatient Pharmacy Clinical Coordinator acts as the Residency Program Director for the acute care PGY1 pharmacy residency program at Good Samaritan Hospital. This position requires familiarity with ASHP residency program conduct, design, and PGY1 accreditation standards.

Minimum Requirements:

Doctorate in Pharmacy (PharmD) Ohio Board of Pharmacy Licensure 1-2 years of experience in Inpatient Hospital Pharmacy

Job Responsibilities:

Coordinate and implement educational training and competency programs for pharmacy staff development and continuing education. Initiate and oversee new clinical pharmacy initiatives. Monitor clinical pharmacy interventions and provide feedback to staff to optimize clinical pharmacy activities. Act as a preceptor for advanced-practice PharmD students, monitoring their activities and evaluating performance. Develop and monitor hospital policies and procedures regarding medication utilization in collaboration with pharmacy leadership and medical staff. Assess pharmacists on the safe and effective use of medication, intervening when necessary to ensure compliance. Provide necessary information to pharmacy leadership for staff performance appraisals. Monitor pharmacy-managed clinical services and consults, providing necessary instruction and education. Participate in the development of the hospital's formulary and related P&T activities. Perform Drug Use Evaluations (DUEs), develop P&T presentations, and discuss findings at P&T meetings. Monitor and manage non-formulary medication use within the hospital. Collaborate with nursing leadership and educators to provide pertinent medication education as needed. Attend physician institute and section meetings as directed. Oversee the residency program, including recruitment, selection, instruction, supervision, mentoring, counseling, evaluation, and advancement of residents. Maintain the Postgraduate Pharmacy Residency Program in accordance with ASHP accreditation standards. Maintain records related to program accreditation. Develop and maintain the practice site and evaluate preceptors. Serve as Chair for the Residency Advisory Committee.
