Clinical Coordinator - Pharmacy

Job Overview:

The Pharmacy Clinical Coordinator provides pharmaceutical care to patients in accordance with federal, state, and local regulations and hospital policy. This position works in collaboration with pharmacy leadership and pharmacy staff to ensure safe, cost effective, and appropriate use of medications. The Clinical Coordinator accomplishes through planning, implementing, evaluating, and monitoring the pharmacy clinical programs, assisting in Drug Usage Evaluations (DUEs), collaborating with pharmacy management and medical staff, and serving as an educator, mentor, and resource for pharmacy staff, students, and residents. Must be able to communicate clearly at all times and be able to prioritize projects to meet goals and established time frames.  

Additionally, the inpatient pharmacy clinical coordinator serves as the residency program director for the acute care PGY1 pharmacy residency program hosted by Good Samaritan Hospital.  The clinical coordinator is therefore well familiarized with ASHP residency program conduct and design as well as the accreditation standards for PGY1 programs.

Job Requirements:

Doctorate Degree in PharmD
Ohio Board of Pharmacy Licensure
1-2 years experience Inpatient Hospital Pharmacy

Job Responsibilities:

Collaboratively, coordinates/implements educational training and competency programs for pharmacy staff development and continuing education. Coordinates/implements new clinical pharmacy initiatives. Monitors clinical pharmacy interventions and provides feedback to staff to optimize clinical pharmacy activities.
Acts as preceptor for advanced-practice PharmD students on rotation, monitoring the student's activities and evaluating performance.
Develops and monitors the hospital's policies/procedures regarding utilization of medications in collaboration with pharmacy leadership and medical staff. Assesses pharmacists regarding safe and effective use of medication. Intervenes when necessary to ensure compliance. Provides necessary, requested information to pharmacy leadership for consideration in the staff's performance appraisals. Monitors pharmacy-managed clinical services and consults. Provides instruction/education, as necessary.
Participates in the development of the hospital's Formulary and related P&T activities. Contributes to the formulary review process by performing Drug Use Evaluations (DUEs), developing P&T presentations, and discussing findings at P&T. Monitors and manages non-formulary medication use within the hospital. 
Works with nursing leadership and nursing educators to provide pertinent medication education as needed or requested. Participates in physician institute & section meetings as directed. 
Oversees the program, including activities related to the recruitment, selection, instruction, supervision, mentoring, counseling, evaluation, and advancement of residents. Maintains the Postgraduate Pharmacy Residency Program within the accreditation standards set forth by the American Society of Health-System Pharmacists. Maintains records related to program accreditation. Develops/maintains the practice site and develops/evaluates the preceptors. Serves as Chair for the Residency Advisory Committee.
