Clinical Coordinator - Social Work

**Position Summary:** The Clinical Coordinator is a Licensed Social Worker who utilizes the expertise in social work to the assigned patient population in collaboration with multidisciplinary team members. The Social Work Clinical Coordinator oversees the social workers and navigator’s effectiveness by coordinating person-centered transitional care plans, resolving barriers, and addressing in-depth psychosocial needs. They coordinate a complex caseload, actively support performance improvement initiatives, and function to provide effective communication between persons, physician practices, the hospital, and the community.
The Clinical Coordinator is responsible for the supervision of the staff members assigned and for the successful operational and administrative functions of the shift. The Clinical Coordinator serves as a resource and assures care is provided in a compassionate, ethical, and knowledgeable fashion and exemplifies the mission, vision and values of Rush Copley and in accordance with policies and procedures. The Clinical Coordinator assures the highest degree of quality care is followed at all times keeping the care and safety of the assigned patient population as a top priority.
**Position Requirements:**
**License:** Current license in Illinois as a Social Worker. LCSW required or will consider candidate with eligibility to test immediately.
**Education:** Master’s Degree in Social Work from an accredited university required.
**Position Requirements:**
+ Ability to perform all job components and serve as a team resource for clinically complex cases within their professional (social work) expertise.
+ Experience related to psychosocial issues, crisis management, conflict resolution, and person-centered planning and care transitions.
+ Skilled educator and communicator. Excellent interpersonal and team building skills, and ability to collaborate effectively with physicians, nurses, and other staff.
+ Previous leadership and process improvement experience strongly preferred.
