Position Summary:
Performs data collection, verification and transmission on patient abstracts.
Reports to: Manager
FLSA: Non-exempt
ESSENTIAL FUNCTIONS:
Retrieves charts from Health Information Management Department using the established records department procedures.
Coordinates all aspects of the patient survey to include distribution, data entry, report generation and submission.
Accurately transmits data and generates reports/graphs in accordance with the procedures in a timely manner.
Maintains strict confidentiality of patient, employee and department data.
Minimum Qualifications:
Minimum Education
High school diploma or equivalent
Graduated from LPN program (Preferred)
Associate's degree (Preferred)
*Years of relevant experience may be substituted for required education.
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Certifications:
Basic Life Support Health Care Provider (BLS-HCP)
