What You Will Do:
Under the direct supervision of the Manager, the Clinical Dietitian conducts nutritional
assessments, plans, directs, and educates the weight management, diabetes and nutrition
patients, as well as special needs patients, including pediatrics. Participates in maintaining the comprehensive clinics in the Center for Coordinated Care,
including, but not limited to Weight Management and Nutrition and Diabetes Self-Management
Education (DSME) program. Participates in meeting requirements for accreditation for DSME program in accordance with
American Diabetes Association standards. Works collaboratively with the interdisciplinary education team. Collaborates with primary
care physicians regarding therapeutic problem-solving of program referrals. Coordinates Weight Management and Nutrition and DSME dietary activities and provides
direct education for individuals and groups. Coordinates and maintains all required
documentation in program participant’s education record and ensures confidentiality. Coordinates efforts to educate and support special needs population groups, such as celiac,
pediatric, or cystic fibrosis patients who have specific nutritional needs. Promotes healthy living and awareness, and obesity and diabetes prevention by providing
educational programs to the community such as community health fairs and support groups. Interviews patients and/or family to obtain information regarding food habits and preferences
for guidance in planning the patient’s diet. Screens medical records for pertinent information and documents as needed per established
guidelines. Calculates diets for weight management patients and other modified diets in accordance with
disease states, and provides diet counseling for all patients on modified diets who require it. Charts appropriately in the patient's medical records after each patient contact. What You Will Need:

Registered Dietitian licensed in the State of Ohio. Certified Diabetes Care and Education
Specialist (CDES), certification in weight management or goal of obtaining certification within
3 years of employment at Firelands.
Experience in nutritional care and education strongly desired.
Successful completion of a one hundred eighty (180) day get-acquainted period.
