Position Summary:
The Bariatric Program Clinical Dietitian provides overall nutrition care to patients. The Clinician ensures the nutritional needs of the individual patients are being met and that all documentation as it relates to the bariatric patient’s nutritional status is complete and accurate. Also plans and administers activities related to patient nutrition care, complying with the standards established by regulatory agencies and the Hospitals. Scope of practice may include both in-patient (hospitalized) and out-patient (community/clinic-based). Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include pediatric, adolescent, adult and geriatric age groups

Detailed responsibilities:
* PROGRAMS - Participate in programs and committees related to the specialty or as directed
* DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshop
* HIPPA - Maintain the privacy, safety, and dignity of each patient by observing client patient confidentiality and by strictly adhering to HIPAA laws
* MISCELLANEOUS - Perform miscellaneous job-related duties as assigned
* COMPLIANCE - Adhere to all established systems and training programs to provide a safe environment. Complies with all OSHA regulations and other local, state and federal regulations
* POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
* LICENSURE - Maintain dietetic registration, state-required credentials and/or licensure and continuing education requirements
* DUTIES - Participate as an integral member of the SRMC Bariatric Surgery program. Evaluating bariatric patients for a minimum of 3 months of visits - conducting an initial individual nutritional assessment and assist in the formulation of weight loss plan leading up to the surgical procedure. The initial assessment will be scheduled in conjunction with the initial Bariatric surgeon consultation.
* PATIENT CARE - Provide appropriate and timely documentation summarizing the nutrition care plan in the EMR, including nutrition assessment, diagnosis, plan, implementation, and progress towards or achievement of established goals in accordance with the Nutrition Care Process
* EDUCATE - Participate in monthly multidisciplinary patient & family education sessions and bariatric support groups
* DUTIES - Conduct group visits with patients and families (which will serve as visits 2 and 3 for preoperative bariatric patients. Depending on patient circumstances, visits 2 and 3 can be individual based on the dietician’s assessment of patient’s needs.)
* KNOWLEDGE - Provide a professional opinion to surgeon on patient’s suitability for surgery
* EDUCATE - Assess educational needs and the presence of patient barriers to learning. Provide inpatient nutritional reinforcement and education for patients on Day 1 or 2 post operatively, taking into consideration any adaptations to teaching methods necessary to meet patient varied needs of age-specific populations as well as cultural, religious, and ethnic concerns.
* FOLLOW UP - Provide appropriate post op follow-up visits in accordance with the bariatric program and patient’s treatment goals. Evaluate patients: 2 weeks, 6 weeks, 6 months, 1 year, 18 mos, 2 years and yearly for a minimum of 5 years post-surgery. (These visits are in conjunction with the post op bariatric provider visits. Patients are referred for outpatient counseling, community, home health services as appropriate.)
* ADVISE - Provide phone advice as needed for bariatric patients
* DUTIES - Participate in monthly multidisciplinary conference on selective patients and programmatic issues
* COMPLIANCE - Active participation in hospital strategic initiatives such as quality improvement and monitoring TJC compliance. Monitor adherence and enforcement of food safety standards
* CUSTOMER SERVICE - Provide excellent customer service to patients, clients and co-workers to ensure patient satisfaction by addressing customer concerns and needs. Conduct rounding to drive patient satisfaction
* BILLING - Maintain clinical productivity standards and accurate record keeping of daily clinical activities, including information for scheduling and billing
* PATIENT SAFETY - Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system
* PATIENT CARE - Promote patient’s independence by establishing patient care goals; teaching patient, friends, identified ley caregiver and family to understand condition, medications, and self-care skills; answering questions
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

* CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days

Physical Conditions:
Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
