Job Overview:

We are looking for a Clinical Documentation Specialist with excellent communication, time management, and organizational skills. This role involves entering orders received from the clinic, entering patient information, demographics, and insurance details, and attaching these to orders and claims. Responsibilities include evaluating documents, collecting and verifying medical information, monitoring records, ensuring compliance with federal laws, and adhering to HIPAA and PHI regulations. This is a work-from-home position with day shifts and no travel is required.

Top Daily Responsibilities: Applying knowledge of medical terminology and procedures to evaluate clinical documents. Collecting medical information from healthcare providers and updating medical records. Verifying the accuracy of patient medical information and obtaining missing information. Monitoring diagnoses, treatments, and follow-up entries in medical records. Ensuring that the content and storage of medical documents comply with federal laws. Comply with all HIPAA and any other PHI regulations Requirements: 1-3 years of equivalent practical experience. Excellent organizational and communication skills. Experience with Data Entry. Ability to keep informed of advancements and regulations in clinical documentation. Preferred Qualifications Experience in Electronic Medical Record (EMR) software Knowledge of medical terminology, coding, and procedures. Knowledge of best practices in medical recordkeeping. Salary and Other Compensation:

The hourly rate for this position is $14-$15, dependent on experience and other qualifications of the successful candidate.

This position is also eligible for Cognizant’s discretionary annual incentive program, based on performance and subject to the terms of Cognizant’s applicable plans.
