Clinical Intake Coordinator LPN
Job Description​

The Clinical Intake Coordinator facilitates the Skilled Nursing/Long Term Care referral process by receiving referrals from multiple referral sources within and without the St. Lukes Health System, performs initial evaluation of referral for Skilled Nursing appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. This position may require travel.
