This is a part time (24 hours/week) position covering Orange County.

Providence Home and Community Care Liaison is responsible for advancing and maintaining referral relationships and referral flow in their assigned territory. Key referral sources include MD Clinics, community hospitals, payor care managers, skilled nursing facilities, assisted living and board and care communities.  This role works closely with our Providence home health and hospice interdisciplinary team.

Required Qualifications:

Education sufficient for clinical licensure for discipline (LPN/LVN) OR Master's Degree in Social Work  Current state professional license or certification appropriate to discipline upon hire. 3 years of experience in clinical discipline.   

Preferred Qualifications:

Liaison, marketing or sales experience. Health care industry experience. Documented prior successful sales account medical management experience. Epic EMR experience.
