This is a per diem/on call position.
This position represents Providence Home Services (HS) at contracted facilities and requires strong communication and interpersonal skills. The role of the Clinical Liaison is to coordinate and arrange home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral, except when there is an onsite HME liaison.

Required Qualifications:
upon hire: Oregon Registered Nurse License. upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties.Preferred Qualifications:
Bachelor’s Degree in Nursing. 1 year Full-time experience in home health or hospice. 1 year Full-time experience in home infusion. 1 year Full-time experience in discharge planning or care management.
