Clinical Manager - Home Care - Full Time

The pay for this position ranges from $39.25-61.36.

Position Summary: The Quality Clinical Manager/Educator (QCME) is responsible for overall performance improvement, QAPI program and activities, all staff education, including OASIS, MatrixCare and clinical competencies for all home health departments.  Responsible for home health education planning, development, and implementation. This includes, but is not limited to, validating required educational needs and competencies for staff, developing training programs and processes related to quality, clinical competence and Matrixcare, ensuring   staff education records are maintained, and keeping leadership team informed of staff education needs. Maintains competence at the expert level in clinical practice. Provides a comprehensive, well organized, and documented orientation for all new employees of Home Health. The QCME acts as a consultant and change agent in the development of standards of care for clinical practices, utilizes and applies change theory in matters related to clinical practice.  Applies interpersonal skill and fosters communication when interacting with staff, patients, and other members of the healthcare organization.  The QMCE is self‐directed in maintaining clinical competencies, pursues continued excellence in clinical practice, is self‐motivated in identifying own strengths and limitations and seeks to improve areas of weakness.  The QCME works collaboratively with nursing managers to assure that standards of clinical practice are met. Education, License & Cert: Graduate of an accredited school of nursing, BSN preferred, AAS with commensurate clinical experience accepted.  Must have PA or NY licensure.  OASIS certification is preferred or should be obtained within 6 months of employment. Experience: A minimum of three years clinical experience and demonstrated abilities in teaching, leadership skills and clinical nursing practice.  Prior or recent experience in public health/home health nursing required.  Prior performance improvement of quality assurance experience required. Essential Functions: 1. Collaboratively develops, coordinates, and evaluates the orientation program for all clinicians, and other staff. 2. Participates in the assessment, maintenance, and development of staff competencies on initial employee hire and annually thereafter. 3. Conducts or coordinates in‐services for new products, equipment, and updated software releases. 4. Collaborates with nursing staff and clinical nurse managers to identify educational criteria to promote high quality nursing care. 5. Collaborates with other educators across the system to develop appropriate educational programs. 6. Organizes, conducts, and evaluates in‐service and continuing education programs for clinicians. 7. Maintains records of educational programs and activities of all home health departments. 8. Encourages development of leadership skills in members of the staff. 9. Provides clinical expertise for administrative decision‐making. 10. Utilizes principles of change theory to effect smooth processes of change. 11. Keeps informed of current clinical practice advancements and research findings and provides for dissemination of research developments. 12. Advance’s nursing practice through evidence‐based practice. 13. Supports performance improvement activities by developing appropriate education related to the topic. 14. Assists in the analysis of “Confidential Quality Reports” data and information and provides risk management and problem oriented educational opportunities as necessary and as requested. 15. Provides education and support and to all staff related to the use and functions to the use of MatrixCare, including orientation of all new staff to MatrixCare. 16. Provide a system of notification (help desk service) related to MatrixCare use and provide solutions to staff who may have IT issues. 17. Monitor all orientation programs weekly as required. 18. Continue to assist in the day‐to‐day supervision of their designated office. Other Duties: 1. Travel for this position may be required. 2.
