The Clinical Manager – Hospice coordinatesand facilitates the provision of quality care and service for a patient at the end of life. This role will demonstrate commitment to hospice and palliative care, excellence in problem-solving, leadership, communication, fiscal responsibility and teamwork with administrators and other members of the health care team to design, evaluate, and improve patient care systems and processes.
The Clinical Manager – Hospice will:
* Participate in the operational, administrative, and personnel responsibilities in providing services in a 24/7 work environment.
* Direct day-to-day operations in accordance with established standards of practice, policy, and regulatory requirements.
* Plan adequate staffing and adjustments on a daily basis.
* Contribute to the development of annual budget.
* Monitor key performance indicators and create an action plan to improve results.
* Oversee the case management process; strategize to improve efficiency and eliminate barriers that impact patient and family-centered care.
* Facilitate and monitor activities related to the delivery of patient care.
* Monitor and develop strategies to address patient satisfaction.
* Participate in event analysis, planning, and corrective actions.
* Administer clinical care to patients, as needed, to maintain clinical skills, to assess particular patient requirements or as support to patient care staff.
* Collaborate with patients, families, and the health care team to support the patient's plan of care.
* Collaborate in planning, implementation, and evaluation of students’ clinical experience.
* Collaborate with Clinical Quality, Education, providers, and Human Resources departments to educate, train and develop staff.
* Facilitate leadership development among staff.
* Maintain membership and involvement in an appropriate professional association that facilitates networking and professional development.
Minimum Qualifications:
* Bachelor’s degree in professional clinical discipline (RN or MSW) from an accredited college or university or active enrollment, Certification in specialty preferred.
* One (1) year of experience in a direct supervisory role preferred.
* Experience in a health care setting with working knowledge of medical terminology, electronic medical records, and billing systems/software, preferred.
* Must have excellent interpersonal, communication and analytical skills.
* Software skills: Electronic medical records, Microsoft Excel, Word, Outlook, PowerPoint, etc.
