Organizes and administers care to bariatric patients, utilizing a patient navigation model through out the medical center and within the community. Provides advanced practice nursing care in collaboration with the medical staff in accordance with the objectives, policies and procedures of the hospital and the New Jersey State Nurse Practice Act. Plans and provides age specific treatments, patient and family teaching and navigation through out the hospital and medical system. Provide community outreach and education programs to increase breast cancer awareness. Qualifications: Required: + Current New Jersey licensure as a Professional Registered Nurse in good standing required. Graduate of an accredited school of nursing required. + 5 years prior experience as a Registered Nurse required BLS (Basic Life Support) Health Care Provider Certification or acquisition within 90 days of hire and maintenance of certification required. + Successful completion of all orientation programs required. Preferred: Certifications and Licenses Required (if applicable): + N/A Scheduling Requirements (non-exempt positions): + Day + Full-Time + 37.5 Hour Work Week Essential Functions: + Performs all other duties as assigned + Oversee and manage all regulatory documentation of MBSAQIP Standards. Prepare and submit reports in a timely manner, maintaining quality of care. + Assist with the coordination of scheduled interdisciplinary Bariatric meetings as per MBSAQIP requirements. + Coordinate all clinical activities for the Bariatric Program and navigate participants through the surgical experience. + Coordinate and provide community outreach and education of Bariatric program. Provide education to medical and nursing staff. + Market and promote the Bariatric navigation program within the medical center and within the community. + Assist with obtaining all necessary medical documentation and coordinate with physicians/clinical staff for preparation of Bariatric surgery + Provide navigation support to patients within the Bariatric program with morbid obesity diagnosis. + Provides a compassionate and supportive environment with prompt and consistent service. + Adheres to patient rights: provides for the confidential treatment of all communications and records. + Demonstrates professionalism with families, visitors, physicians, co-workers and supervisors. + Supports teamwork by cooperative problem solving through participation in meetings, projects, etc. + Utilizes principles of continuous quality improvement in all work situations to assess, measure and improve organizational and department functions + Organizes work sets priorities with a minimum of supervision and seeks guidance as appropriate + Makes decisions that are timely and consistent with department, objectives, policies and procedures.
