Responsibilities This position is the primary liason for the continuity of care and social support systems for patients and their families throughout their health care experience. Develops education and training programs for patients, physicians and care staff on the appropriate clinical, community requirements and programs. Guides cancer patients through their course of clinical care working with a variety of medical practitioners, facilities, and community resources. Works with community advocates for the patient. Tracks and trends data for the program reporting requirements. Adheres to policies, procedures and regulations to ensure patient safety and compliance. Qualifications Job Requirements: Bachelor in Nursing or equivalent required. 1-2 years Navigator experience preferred, excellent communication skills Preferred Requirements: Master’s in health related field preferred
