 Position Description: Clinical Nurse Navigator - RN 

 BayCare Health System is currently seeking a Registered Nurse to join our outstanding and compassionate Nursing Administration team at St. Joseph’s Hospital as a Clinical Nurse Navigator. The Clinical Nurse Navigator responsibilities include:

Serving as the coordinator for the care of patients in their specific specialties.Providing support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience.Facilitating integrated care through communication with strategic members of the interdisciplinary team.Collaborating to provide staff education and working closely with patients, families, and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.Coordinating efforts between physicians, healthcare administration, family, community resources and staff while tracking the progress of the patient throughout their continuum of care.Promoting evidence-based clinical care that exemplifies the pursuit of clinical excellence.

Qualifications:

Certifications and Licensures:

BLS (Basic Life Support) Certification is requiredRN (Registered Nurse) license is requiredACLS Certification if required by departmentNurse Specialty certification is preferred or certification in related specialty must be obtained within the first 6 months if not current.

Education:

Bachelors degree in Nursing is required

Experience:

2 years of RN experience in acute care is required. Cardiac background is preferred.
