The Clinical Outcomes Specialist performs extensive medical record review and concurrent chart data abstraction on all regulatory/registry-required clinical indicators for assigned patient groups. Selects and collects appropriate data from patient charts relevant to the core measures set and disease-specific indicators. Trains and educates others in the area of core measurement requirements, as it pertains to quality and outcomes initiatives. Provides directions and guidance to others regarding practice while serving as a resource and mentor. Utilizes standards, guidelines and pathways for care delivery.

Job Responsibilities
              

Performs extensive medical record review and concurrent chart data abstraction on all regulatory/registry-required clinical indicators for assigned patient groups. Selects and collects appropriate data from patient charts relevant to the core measures set and disease-specific indicators.  Delivers timely and comprehensive outcomes reports to QI committees, clinical and administrative executive leaders.  Works concurrently and collaboratively with physicians, clinicians, and other individuals involved in the quality improvement initiatives and core measures to ensure standards of quality are met.  Participates in educational programs of staff and physicians to increase performance levels in quality improvement initiatives and core measures process.

Additional Requirements:

Certification - Advanced Practice Registered Nurse Board Certified within 3 months of hire (applicable to RNs only) Licensure - Registered Nurse License in the Commonwealth of Virginia upon hire; for non-RNs, must be  registered in one of the following: RCIS, RCES, RTRCV, or RTRVI upon hire Experience - 3 years experience in a clinical setting Education - Bachelor's Degree in Nursing BSN, or graduate of an accredited school of Cardiovascular Technology or Radiology Technology years of specialized training in a hospital/healthcare.
