Responsibilities
Carry out day-to-day duties and responsibilities as they pertain to providing our client’s Specialty Pharmacy with the contracted servicesAct as a resource to the client’s clinical and operational pharmacy staffAssist the physician and other healthcare professionals involved in planning and implementing patient care to ensure safe and effective drug therapyConduct initial and ongoing assessments of specific patient drug and disease information to identify real and potential drug-related problems or needs. Documenting this in CarepathRx’s Patient Therapy Management Software Solution - DromosPTMAssist the physician and office staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapyUnderstand billing, reimbursement, case management, and other third-party payor requirements/implications for pharmacy products and servicesAssist the physician and office staff in completing prior authorizations for their specialty patientsUnderstand the workflow of various EMR systems and DromosPTM and other systems as neededMonitor the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient informationCommunicate the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate personnelMake appropriate recommendations that are designed to enhance the outcomes associated with drug therapyProvide consultations and answer inquiries from patients, families, physicians, and other healthcare professionalsMaintain thorough and appropriate documentation in the patient/client record (EMRs, DromosPTM, etc.) of all clinical activities and communication with patients, physicians, and other healthcare professionalsDemonstrate the ability to work effectively and maintain expected productivityIf applicable, Process and checks all medications prior to dispensing from the pharmacyMaintain pharmacy records in accordance with applicable laws and regulationsCarry out all responsibilities in accordance with CarepathRx’s Core Values Experience a strong knowledge base in Specialty pharmacyCapable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionalsAttend regular staff meetingsEstablish positive working relationships with internal and external customers Meet expectations for customer satisfaction and anticipate customer needsHandle customer complaints in a fair and empathetic mannerMaintain confidentiality of patient informationParticipate in CarepathRx’s Performance Improvement ProgramPromote CarepathRx culture by providing quality, comprehensive services to all customers through a team approachAssist with the development and enforcement of policies and procedures that promote cost-effective, appropriate, and safe medication useSupport the pharmacy's pharmacoeconomic effortsReview the use of targeted medications for compliance with current pharmacoeconomic programsAssist with pharmacokinetics consult service and drug dosing per organizational protocol. Skills & Abilities
Proficiency in Microsoft Suite and pharmacy software programActive Pharmacist license in good standing with the Board of PharmacyExperience in specialty pharmacy. Health system specialty pharmacy preferredStrong clinical knowledgeDemonstrate clinical competence on an ongoing basisGood verbal and written communication skillsAbility to work in a team environment and cooperate with all departments within CarepathRxMeet any continuing education requirements of the company’s clinical program, as defined within the company’s policy and proceduresAbility to stay abreast of developments in the field of specialty pharmacy, self-injectable, and oral therapiesAbility to handle challenging situations in a professional and proficient manner utilizing available resources and critical thinking skillsMust be flexible with work schedule and on-call duty
Qualifications
B.S. degree in Pharmacy or Doctor of Pharmacy (Pharm D.) required2+ years of previous experience in specialty pharmacy services is preferredCertified Specialty Pharmacist (CPS) preferred or willing to obtain this certificationMust be flexible with work scheduleDemonstrate ability to solve problems and supervise technical and support staffGood organizational and time-management skillsClear and effective written and verbal communicationCommunicates in a timely manner and is capable of active listeningReceptive to new ideas and viewsAdaptable to fast-paced and often evolving environmentPharmacist Licenses and CertificationsLicensed in the state of Tennessee and Alabama or willingness to obtain license quickly (Multiple state licenses preferred)Willingness to get licensed in more states as neededAbility to perform in a remote setting that requires sitting at a desk 90% of the time
