Position Summary: Clinical Pharmacist – Anticoagulation (Troy, Albany, and Clifton Park)
Position Purpose:
The Anticoagulation Clinical Pharmacist is responsible for Anticoagulation Clinic Services. The pharmacist assists in caring for patients on anticoagulation therapy to achieve optimal desired outcomes.

Provide direct oversight and guidance for the Anticoagulation Clinic Services.Perform drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients. Provide appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocols and established best practice guidelines.Observe, record and report patient’s condition and reaction to drugs and treatments to physicians. Prescribe medications as outlined in provider approved CDTM.Oversees appointment bookings and ensures preferences are given to patients in emergency situations. Maintain timely flow of patients.Respond to and refer incoming telephone calls (Triages). Instructs patient and family regarding medications and treatment instructions as directed by physician.Educate patients and families regarding anticoagulation therapy.Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes.Communicate effectively with prescribers referring patients to the Anticoagulation Clinic.Maintain competency in anticoagulation care. Serve as an active patient advocate regarding anticoagulation therapy. Use a team approach to working with staff and othersActive Participation in the training of St Peters PGY-1 Pharmacy residents and PharmD studentsParticipates in performance improvement initiatives and research as appropriate.Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.Responsibilities include disease management within institution approved collaborative practice agreements (anticoagulation), referral-based Comprehensive Medication Review with recommendations to physicians, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and costParticipates in performance improvement initiatives and research as appropriate.Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant.Participate in various components of the Value Based Pharmacy Care focusing on the following:Prescription drug adherence (diabetes, RAS antihypertensive, statin);Statin use in persons with diabetes ("SUPD")Cost savings opportunitiesComprehensive medication review (CMR) and resulting targeted medication interventionsMinimum Qualifications:
New York State Pharmacist Licensure, either by examination or by reciprocity.Demonstrated competence in providing direct patient care.Experience Preferred:
PGY1/PGY2 Residency and/or Fellowship Training in Ambulatory Care or 5 years’ experience in direct Ambulatory Care.Board Certified Ambulatory Care Pharmacist (or eligible within 1 year)Must meet requirements for Collaborative Drug Therapy Management in NYS Pharmacist CredentialsExcellent communication, interpersonal and organizational skillsUnderstanding of insurance practicesUnderstanding of telehealth practicesPrevious anticoagulation management experience preferred.Self-motivated, detail-oriented individual able to work with minimum supervision.Pay Range: $56.00 - $72.30
Pay is based on experience, skills, and education. The pay range may also vary within the stated range based on location.
