Job Summary:
The position requires an active CA state license.
As a remote Clinical Pharmacist at Included Health, you will ensure that patients' pharmacy-related needs are met through industry-leading interventions. You will collaborate with a multidisciplinary team of providers, such as nurses, social workers, behavioral health specialists, and care coordinators, to deliver high-quality, evidence-based care to members across the United States.
Your role will focus on supporting individuals enrolled in Included Health's Case and Care Management and Virtual Primary Care Programs, along with other product offerings. You will provide pharmacy benefits navigation, treatment guidance, medication support, and chronic disease management. Interventions will be delivered either directly by Included Health or through local care, community resources, or other plan benefits, with your expertise and collaboration driving optimal patient care.
Responsibilities:Complete comprehensive Medication Reviews and Medication Reconciliation post-discharge for our Virtual Primary Care and our complex Care and Case Management members.Standing Orders and Medication Therapy Management: Collaborate with the Virtual Primary Care team to enhance chronic disease management outcomes for patients. You will recommend or initiate therapies and work longitudinally with patients.Be the subject matter expert: Support the greater clinical team by consulting on pharmacy related questions and our members on pharmacy benefits navigation.Develop and present educational content to members through group classes, and provide training and presentations for our our teams.Support our overall pharmacy strategy in collaboration with the pharmacy team and other key stakeholders across the company. You will develop pharmacy interventions, assess internal and external pharmacy solutions, and contribute to strategies that improve the health of entire populations.Partner with clinical leadership to deliver innovative, high-quality care by developing and improving pharmacy interventions, assessing internal and external pharmacy solutions, and contributing to ROI methodology.Qualifications and Requirements:Doctor of Pharmacy (PharmD) from an accredited program.Completion of a PGY1 Pharmacy Residency program.Active pharmacist license in good standing with the California State Board of Pharmacy.Willingness to obtain additional state licenses. 5+ years of experience in primary care, ambulatory care, or a similar clinical setting. Clinical experience in oncology and at an integrated health care system or health plan is valuable.Board Certification in Ambulatory Care or Pharmacotherapy (BCACP or BCPS).Schedule: Work standard business hours in the Pacific Time Zone, Monday through Friday, between 8:00 AM and 6:00 PM PST, with flexibility in start and end times.Physical/Cognitive Requirements:Prompt and regular attendance at assigned work location.Capability to remain seated in a stationary position for prolonged periods.Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer \/ laptop) may be required.Capability to work with leadership, employees, and members in an appropriate manner.The United States new hire base salary target ranges for this full-time position are:
Zone A: $118,640 - $167,580 + equity + benefits Zone B: $136,436 - $192,717 + equity + benefits Zone C: $148,300 - $209,475 + equity + benefits Zone D: $154,232 - $217,854 + equity + benefits
This range reflects the minimum and maximum target for new hire salaries for candidates based on their respective Zone. Below is additional information on Included Health's commitment to maintaining transparent and equitable compensation practices across our distinct geographic zones.
Starting base salary for the successful candidate will depend on several job-related factors, unique to each candidate, which may include, but not limited to, education; training; skill set; years and depth of experience; certifications and licensure; business needs; internal peer equity; organizational considerations; and alignment with geographic and market data. Compensation structures and ranges are tailored to each zone's unique market conditions to ensure that all employees receive fair and competitive compensation based on their roles and locations. In addition to receiving a competitive base salary, the compensation package may include, depending on the role, the following:
