Job Description:
The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective, and cost effective pharmaceutical care in the scope of their particular assignment. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.Scope
PHARMACY SPECIFIC
1. Dispensing: Interprets physicians' orders and compounds and dispenses medications within regulatory guidelines. Reviews patients medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug-drug interaction. Consults with medical staff regarding use of medications and potential drug interactions or allergies. Prepares and maintains all necessary records on prescriptions, patient charges and inventory. Ensures formulary and policy compliance.
2. Oversight: Supervises and directs activities of interns, technicians, residents, and support personnel as required.
3. Clinical: Completes specialized therapeutic consults and pharmacologic interventions in the areas of kinetics, anticoagulation, antimicrobial stewardship, pain, renal dosing, medication history documentation, therapeutic substitution, etc. as defined by policy and scope of practice.
4. Education: Counsels medical staff, nursing, and patients and handles complex problem resolution.
5. Quality and Regulatory: Assumes responsibility for ensuring all medications are stored, compounded and dispensed in compliance with practice standards and regulatory requirements including USP, DEA, State Board and other applicable state and federal laws.
6. Professionalism: Performs in an efficient, professional manner to meet pharmacy profitability and productivity targets. Supports inventory and stewardship initiatives. Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns. Participates in and leads quality and patient safety initiatives. Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff, and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedback.
7. Provides orientation and training for new associates and students as requested by management.
8. Performs other duties within scope when assigned by management or when needed to provide patient care services.
