Provides pharmaceutical care to home infusion patients.  Coordinates care with patients, family/caregivers, physicians, nurses, and other pharmacy personnel.

Required qualifications:

Education to meet certification, license or registration requirement. California Pharmacist Registered upon hire 2 years of experience in infusion pharmacy or IV therapy in a hospital setting.
