Qualifications

EDUCATION

Minimum Required: BS in Pharmacy.Preferred: Pharm D. 

EXPERIENCE

Required: At least one year’s previous experience in a hospital setting as a Clinical Pharmacist

SKILLS & OTHER ABILITIES

Preferred: Good communication skills and ability to relate effectively to hospital staff, department heads, administration and medical staff

REQUIRED CERTIFICATION/ LICENSURE/ REGISTRATION

Current & Valid Florida License as Registered Pharmacist

Description

Position Summary

The Clinical Pharmacist is responsible for providing pharmaceutical care to patients; a working knowledge of pharmacokinetics by assisting physicians and staff pharmacist in drug-dose relationships; for providing the hospital with a safe and effective way of monitoring drugs to avoid the potential for any toxic reaction; for provision of Nutritional Support functions. Participates in on-going Performance Improvement programs and related clinical pharmacy practices as per Joint Commission and ASHP guidelines. The Clinical Pharmacist verifies/proposes medication orders in electronic health record.
