Clinical Pharmacist - MTM

The Clinical Pharmacist provides clinical pharmacy services to patients in order to optimize medication related healthcare outcomes. The services may include a broad spectrum of Medication Therapy Management (MTM) services including, but not limited to, Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions of Care management.

Responsibilities

Deliver MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy and to improve clinical outcomes.

Reviews patients’ medication profiles and applies evidence based medicine and national guidelines when creating recommendations to health care providers for optimizing the patients’ medication regimen.

Assist with managing medication costs through cost-effective interventions which may include, but are not limited to, formulary or therapeutic interchanges, pill splitting opportunities, combination products. and assessing unnecessary and/or duplicate medication therapies.

Efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, patient educational gaps, ineffective medications, inappropriate doses, adverse drug reactions, adherence concerns and opportunities for medication synchronization.

Generate deliverables to include: Personal Medication List (PML) through comprehensive medication reconciliation, patient-specific Medication Action Plan (MAP) and written/ verbal communication and feedback to healthcare providers. Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams.

Serve as an advocate for and explain the benefits of MTM services to patients, family members and providers.

Ensure patients and family members have a positive experience by providing a high level of customer service by always responding promptly, professionally, and politely.

Follow up with patients and providers to check on progress toward meeting drug treatment goals.

Serves as a drug information resource to Patients, Providers and other members of the healthcare team.

Provide other clinical pharmacy services as directed by manager.

Meet productivity and performance expectations set forth by manager.

Qualifications & Requirements

High speed internet connection through cable broadband or fiber optic service provider

B.S. degree in Pharmacy or Doctor of Pharmacy (Pharm D.) from an accredited school of pharmacy required.

2+ years experience as a clinical pharmacist

Medication Therapy Management experience, preferred

Current and unrestricted pharmacist licensure in your state

Willingness to reciprocate pharmacist license to additional states as needed

Excellent interpersonal and customer service skills

Effective communications skills, both written and verbal

Strong IT and troubleshooting experience

Proficiency with office technology tools, to include MS Office 2000 or later (esp. Excel, and Word), and Google G-Suite

Physical Demands

May sit, stand, stoop, bend, and walk intermittently during the day.

This role requires typical office-based physical activities: watching a computer monitor, use of a keyboard, occasionally repositioning light desk equipment, and speaking on a telephone for extended periods of time.
