The Clinical Pharmacist for Transplantation & Cellular Therapy will help provide comprehensive pharmaceutical care to the oncology population at Sylvester Comprehensive Cancer Center. Work related activities may include drug selection, drug information, regimen creation and ordering, monitoring and toxicity management, formulary management, protocol and pathway development, Beacon review, outcome evaluations, and patient or staff education/counseling. The Clinical Pharmacist is key to identifying quality initiatives to improve patient outcomes, continued overview of policies, and/or guidelines, and other clinical pharmacy initiatives. Clinical Pharmacists in this setting work with a multidisciplinary team to implement the highest quality of care for our patients. CORE JOB FUNCTIONS + Supports the Transplant and Cellular Therapy clinical services that reflect the vision, mission, and values of University of Miami Hospital and Clinics/Sylvester Comprehensive Cancer Center. + Provide expert medication management, including monitoring and optimizing the use of high dose antineoplastics, immunosuppressants, anti-infectives, anti-seizure medications, anticoagulants, and chemotherapeutic agents. + Individualizes medication regimens while accounting for pharmacodynamic and pharmacokinetic variations in drug absorption. + Conduct thorough medication reviews, ensuring safe and effective medication use across all phases of transplant care in the inpatient and outpatient setting. + Participating in interdisciplinary rounding, offers chemotherapy and medication counseling to patients and caregivers, focusing on side effects, medication administration, and therapy adherence. + Assist in symptom management, including supportive care and pain/palliative care, with a focus on optimizing graft-versus-host disease (GVHD) management, and appropriate anti-infective therapy. + Contribute to therapeutic drug monitoring for various agents, such as immunosuppressants, chemotherapeutic agents, and anti-infectives. + Assist in the facilitation of safe post-transplant vaccine administration. + Contribute to the development and revision of evidence-based clinical policies, guidelines, and protocols related to stem cell transplant. + Collaborate in institutional and multidisciplinary research, focused on improving transplant-related outcomes and advancing clinical practice. + Involvement in external presentations, publications, and professional organizations is strongly encouraged. + Participates in collaborative departmental subcommittees, including but not limited to Pharmacy and Therapeutics Committee and Medication Events Committee. + Serves as preceptor for and maintains active involvement in the PGY-2 Oncology residency program. + May provide precepting mentorship for PGY-1 pharmacy residents and pharmacy interns. + Adheres to University and unit-level policies and procedures and safeguards University assets. CORE QUALIFICATIONS + PharmD is required; consideration will be given to all candidates who have been grandfathered by the state of Florida to keep their licenses active/current if obtained before the changes to the Pharmacists’ licenses (PharmD requirement) took place, this also includes candidates from other countries that have obtained a Florida license. + Requires current licensure in good standing in the State of Florida and a degree in Pharmacy from an accredited College of Pharmacy + PGY-1 residency certification + PGY-2 Hematology/Oncology Residency or Board Certification in Oncology Pharmacy required. + Minimum 3 years of relevant experience The University of Miami offers competitive salaries and a comprehensive benefits package including medical, dental, tuition remission and more.
