Clinical Pharmacy Coordinator

What you'll do The Pharmacy Clinical Coordinator will be responsible for coordinating clinical pharmacy services, ensuring the safe and effective use of medications, and optimizing patient care outcomes. This role requires a deep understanding of pharmacotherapy, excellent communication skills, and the ability to work collaboratively with healthcare professionals. The Pharmacy Clinical Coordinator will also be involved in developing and implementing clinical protocols, conducting medication therapy management, and providing education to patients and staff. The successful candidate will have a strong background in clinical pharmacy, a commitment to patient safety, and the ability to lead and inspire a team. The Pharmacy Clinical Coordinator maintains a neat, professional, well-groomed appearance and wears identification badge at all times as observed by the Supervisor. Performs work within specified times and prioritizes the order in which work needs to be completed for both patient safety and patient satisfaction. Adapts positively to frequent interruptions and changes in workload and/or work schedule, as observed by the Supervisor and co-workers. The Pharmacy Clinical Coordinator works cooperatively with all network staff including pharmacy management, providers and Network or Entity Leadership. Coordinate and oversee clinical pharmacy services Develop and implement clinical protocols and guidelines Conduct medication therapy management and review patient medication regimens Collaborate with healthcare professionals to optimize patient care Provide education and training to patients and healthcare staff Monitor and evaluate the effectiveness of medication therapies Ensure compliance with regulatory and accreditation standards Participate in multidisciplinary team meetings and case reviews Conduct clinical research and contribute to scholarly publications Manage and mentor pharmacy staff and interns Develop and maintain clinical documentation and records Assist in the development of pharmacy policies and procedures Provide drug information and support to healthcare providers Participate in quality improvement initiatives Ensure the safe and effective use of medications Conduct patient assessments and develop individualized care plans Monitor for adverse drug reactions and interactions Participate in formulary management and medication selection Support the implementation of electronic health records and clinical decision support systems Stay current with advancements in clinical pharmacy practice What we need from you Education: 5 year degree in Pharmacy or Pharm.D. Must maintain all requirements including continuing education as mandated for by the New York State Board of Pharmacy, required Completion of a Pharmacy Residency Program, preferred Experience: 3 years Clinical pharmacy experience, preferred Licensure/Certifications: Licensed and currently registered in the state of New York to practice Pharmacy, required Skills: Communication Organizing/Categorizing Records Time Management Decision Making Critical Thinking Problem Solving Manage People Leadership Team Building Physical Requirements: Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $125,465.60/Yr. Pay Range Maximum: USD $163,105.28/Yr.
