Clinical Pharmacy Specialist: Ambulatory Hematology & Oncology - $10,000 Sign-On Bonus
Job Description

As a Clinical Pharmacy Specialist, you will provide high-level patient care for a specialized population, ensuring adherence to professional standards. You will consult with patients, families, and medical staff, participate in research and drug management programs, publish data, present at meetings, train pharmacy residents and students, oversee clinical services, and mentor clinical pharmacists.

Clinical Pharmacy Services

Participate in direct patient care with medical or surgical teams.Assist with therapeutic decisions, including drug selection, dosing, and monitoring.Develop patient care plans in collaboration with the medical team.Recommend therapy or alternatives based on medical literature and clinical experience.Provide clear, succinct verbal or written recommendations.Consider age, weight, and organ function (e.g., renal, hepatic) when evaluating therapies.Interview patients and conduct physical assessments to determine medical history and therapy response.Interpret laboratory and patient-specific data to develop treatment plans and monitor therapy response.Apply pharmacokinetic principles to achieve safe, effective, and economical regimens.Respond to therapeutic queries from healthcare providers.Identify and correct drug-related problems, such as ineffective therapy or adverse reactions.Establish procedures to detect and avoid significant drug interactions.Perform basic cardiac life support and manage drug therapy during medical emergencies.Ensure continuity of care by collaborating with other health professionals and organizations.Counsel patients and caregivers on medication administration, potential adverse reactions, and storage.

Drug Use, Outcome and Formulary Management

Ensure and enforce compliance with clinical protocols and guidelines among multidisciplinary care providers.Collaborate with the Coordinator of Drug Use and Outcome Management to identify and establish programs that improve drug therapy outcomes and patient care quality.Work with the Coordinator of Drug Use and Outcome Management and Formulary Management to implement and monitor drug use management programs.Participate in the Medication Use Evaluation Program by designing drug use criteria, collecting patient data, and providing accurate analysis and reports.Assist in clinical pathway development and managed care initiatives, such as disease management programs.Contribute to formulary management, including developing drug monographs, treatment protocols, policies, algorithms, and guidelines.Document pharmaceutical care interventions as part of the departmental documentation program.Company Description

Education:

Required: Bachelor of Pharmacy from an accredited institution (American Association of Colleges of Pharmacy).Preferred: Master of Science, PhD, or Doctor of Pharmacy from an accredited institution.

Experience:

Required: Minimum of 5 years of clinical pharmacy experience with a Bachelor’s degree.Required: Completion of a Pharmacy Practice Residency or at least 3 years of clinical pharmacy experience in Oncology.Preferred: Completion of a Specialty Residency in Oncology. (Note: The 5 years of clinical experience is in addition to the 3 years mentioned here.)

Licensure:

Required: Licensure by the Maryland State Board of Pharmacy.

Certifications:

Preferred: Certification from the Board of Pharmaceutical Specialties.Preferred: Certification as an Anticoagulation Provider if working in the Anticoagulation Clinic.

Skills and Abilities:

Proficiency in personal computer operations and applications (e.g., spreadsheets, database management, word processing).Demonstrated ability to impact pharmaceutical care outcomes.Strong planning, organizational, and project evaluation skills, including delegation.Ability to stand for extended periods and occasionally lift supplies and equipment.Highly effective written and verbal communication skills for interaction with clinical staff, upper management, and other personnel across multiple departments.Additional Information
