Clinical Pharmacy Specialist- Ambulatory Oncology

Job Description

Implements, coordinates, and evaluates all clinical Pharmacy services. Assists and conduct Medication Usage Evaluations and Research projects, develop policies/guidelines, collaborates with the medical staff, and establishes criteria for the safe, effective and appropriate use of medication.

Job Responsibility

Implement CDTM practice with the oncology team and provide patient education on new chemotherapy regimen.  Serve as a clinical pharmacy liaison to the oncology team on medication related matters to facilitate patient care Serve as a Primary Preceptor for LIU pharmacy students for the ambulatory oncology rotation Actively participate in the Cancer Institute’s P&T and assist in developing/updating oncology clinical guidelines and oncology medication-related policies, and involve in oncology formulary management.  Develop drug monographs for new oncology medications to be presented at the P/T for formulary consideration  Actively participate in the Cancer Institute’s Medication Safety Committee meetings and assist in developing/updating oncology clinical guidelines and oncology medication-related policies Actively participate in the Northwell Health Ambulatory P/T and present new non-oncology medications for formulary review Provide in-services to pharmacy staff and nursing staff on new antineoplastic and non-antineoplastic agents  Attend other department meeting as needed Contribute to staff performance evaluation from a clinical perspective as needed In collaboration with the Cancer Institute’s physician and nursing leadership, develop evidence-based medication use guidelines and policies for the Cancer Institute Collaborate with the oncology team to conduct clinical and QI research projects, and present the findings at national cancer conferences (e.g. ASH, HOPA, ASCO) and/or publish manuscripts in oncology medical and pharmacy peer-reviewed journals Actively involve in hematology/oncology societies, including HOPA, and volunteer services, and contribute to the field of hematology/oncology pharmacy

Job Qualification

Completion of PGY2 Oncology Pharmacy Residency or equivalent experience (PGY1 Pharmacy Residency plus 2 years of clinical experience in adult oncology or 5 years of clinical experience in adult oncology plus BCOP) preferred   PharmD required BCOP preferred (if not BCOP, goal to obtain within 3 years of this position appointment) Active member of HOPA Excellent communication skills
