Clinical Pharmacy Specialist - Anticoagulation Clinic

The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Clinical Pharmacist Specialist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.

Required qualifications:

Bachelor's Degree in PHARMACY from an accredited School of Pharmacy. Or Ph.D. In PHARMACY from an accredited School of Pharmacy.     Coursework/Training and Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience Or equivalent educ/experience Upon hire: Oregon Pharmacist License  Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice Upon hire: Pharmacy preceptor license in applicable state as applicable TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
