Clinical Pharmacy Specialist in Oncology
*Description* Summary: The primary functions of the Oncology Pharmacy Specialist are developing and providing quality pharmaceutical care to all oncology/hematology patients, including monitoring drug therapy, provision of clinical and drug information, participating in patient rounds in designated areas, supervises the preparation on chemotherapy and maintaining and developing clinical protocols. The Oncology Pharmacy Specialist will oversee and manage the chemotherapy medication formulary (via co-chairing pharmacy and therapeutics committee). Responsibilities: 1.Develops & maintains a clinical practice in the oncology setting, cooperating with medical & nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents and design of a regimen to maximize therapeutics benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieved desired outcomes. 2.Provides drug and clinical information to other healthcare professionals in response to specific questions relating to new evolving issues related to the care of patients with cancer. Provides professional education regarding new drugs, biologics or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, residents, students and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes. 3.Performs pharmacokinetic dosing and monitoring for all patients on drugs where therapeutic levels are reported. 4.Provides visual, spoken and written clinical education and training to pharmacy students, pharmacists, clinical pharmacists and residents in the Department of Pharmacy. 5.Provides managerial responsibilities for the operations of the Cancer Center satellite, including process improvements and maintaining oncology formulary. Designs, supports and implements policies to provide effective and efficient use of resources for ensuring that budget, quality and value targets are met in collaboration with the Acute Care Manager. 6.Develop, review, assist and maintain, in collaboration with members of the Department of Hematology/Oncology various protocols and regimens within the computerized physician order system. Other Information: * Bachelor of Science degree in Pharmacy from an approved college of Pharmacy required. Doctor of Pharmacy (PharmD) degree preferred. * Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. (Must obtain a registered license within 3 months of employment.).Board Certification in Oncology required within 1 year of employment * Previous oncology pharmacy practice experience (3 to 5 years) in institutional environment and/or completion of an ASHP-accredited hospital (PGYI & PGY2) residency with a focus in oncology * Member of the local, regional, and national professional organizations preferred. * Demonstrated knowledge and compliance with all State, Federal, and malpractice laws of pharmacy practice. * Excellent communication and interpersonal skills and experience working on interdisciplinary initiatives across all levels of organization Working Conditions: Manual: Some manual skills/motor coord & finger dexterity Occupational: Some occupational risk Physical Effort: Medium to Heavy effort.
