Clinical Pharmacy Specialist Transitions of Care Discharge Lounge Clinic
The Butterworth Discharge Lounge clinic is a new service line since June 2024. This clinic focuses on providing comprehensive medication reviews and care coordination related to medications impacting hospital re-admissions. This position will work with a variety of disciplines within Butterworth hospital to ensure patients have all the necessary education, medications, and follow-up appointments for a successful transition from inpatient to outpatient. The pharmacist will utilize a collaborative practice agreement with the patient's medical group primary care provider or hospitalist to make necessary medication adjustments. Knowledge of formularies, manufacturer coupons/patient assistance programs, and problem-solving community pharmacy billing issues are ideal skills for the candidate to possess. Overview: This position will support Corewell Health patients with Transitions of Care (TOC) pharmacy services through the discharge process. Comprehensive Medication Reviews (CMRs) and TOC clinical services are completed to support patient safety, medication access, provider and patient satisfaction, and the total cost of care for Corewell Health and our patients. Clinical Pharmacy Specialists within the Discharge Lounge Clinic will work closely with providers under a Collaborative Practice Agreement (CPA) to evaluate and optimize medication regimens for mutual patients allowing for a seamless transition to home until their post-hospital appointments. They will provide guideline and evidence based Chronic Disease and Medication Therapy Management directly to patients. This position will assist patients with cost, adherence, and lifestyle barriers related to their medications and they will work closely with inpatient and outpatient staff to coordinate care. This position will also navigate EPIC and utilize features within the EMR daily. Essential Job Functions: + Provide comprehensive medication reviews (CMR’s), transitions of care (TOC) services, chronic disease state management, and/or medication therapy management directly to patients within the Discharge Lounge Clinic. Pharmacists may work under a collaborative practice agreement with providers to provide medication management directly to their patients. + Build relationships and collaborate with inpatient and outpatient providers and staff by living our values to promote trust and gain referrals within the clinic. + Follow Corewell Health standard work, policies, and procedures to gather, order, and interpret patient medications, vitals, and labs during visits. Locate and use clinical data within the EMR to assist with developing TOC plans. + Assist system in monitoring and improving upon system quality and value measures or initiatives including, but not limited to, MTM, A1c, Hypertension, Adherence, and Readmissions. + Help patients and staff navigate formularies and other programs to assist in lowering the cost of care for patients and the system. + Engage in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Complete required continuous training and education, including department and Spectrum Health specific requirements on a timely basis. + Precept pharmacy residents and students independently as assigned.
