POSITION SUMMARY

The Clinical Registered Dietitian II will be responsible for assessing the nutritional status and developing evidence-based nutrition care plans for patients across the lifespan with various conditions including but not limited to Type 2 diabetes, hyperlipidemia, hypertension, metabolic syndrome, and weight management. This individual will work collaboratively with the integrated care team of behavioral health consultants registered nurses, care coordinators, medical assistants, and primary care providers. The dietitian will be responsible for establishing SMART goals with their patients and establishing follow-up appointments as needed. The Registered Dietitian II will be responsible for training and onboarding new RD’s on integration and conducting efficient nutrition consults. The Registered Dietitian II will also be responsible for attending and participating in the monthly provider meetings and providing nutrition updates. The Registered Dietitian II will be responsible for precepting Dietetic interns. The RD II effectively prioritizes patient care and readily assists newer RD’s in the field with questions and concerns. This position is essential to Adelante’s team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.
