You can grow your career here while enjoying first-class perks, benefits and commitment to diversity and inclusion. **Overview** The Clinical Review & Medication Therapy Management Pharmacist will be responsible for completing prior authorization reviews and assisting in formulary management, pharmacy and therapeutics committee activities, communication with physicians and pharmacists, and communication with staff on departmental policies and procedures. On call duties are a requirement of the job. They will also be responsible for completing work to support Independent Health’s Medicare MTM program and pharmacy quality initiatives for our Medicare, Commercial and MediSource lines of business as needed. This will be done through interactive telephonic and/or claims review of targeted patients' prescription, medical and laboratory claims history to identify opportunities to improve therapeutic outcomes and reduce the risk of adverse events. The Pharmacist will contact identified patients to provide education and coaching as well as to identify potential opportunities to improve patient care. Additionally, they will work with prescribers to facilitate appropriate changes to medication regimens. **Qualifications** + Bachelor’s degree required. PharmD preferred. Licensed to practice pharmacy in the state of New York required. + Five (5) years of pharmacy experience required. Professional experience providing pharmaceutical care services or MTM experience preferred. + PC and windows skills required. Proficiency with pharmacy computer systems preferred. + Excellent written and verbal communication skills with experience communicating with other health care professionals. + Knowledge and skill to conduct a thorough drug regimen review. Strong clinical knowledge base kept current through educational endeavors and professional affiliations, strong comprehension of pharmaceutical managed care issues. + Proven examples of displaying the IH values: Passionate, Caring, Respectful, Trustworthy, Collaborative and Accountable. **Essential Accountabilities** + Assist in formulary management through knowledge of all aspects of the prior authorization process and clinical protocols, policies and procedures required to administer the pharmacy benefit with consistency and expediency. + Prepare communication as required. + Demonstrate knowledge of clinical protocols, policies and procedures required to perform drug regimen reviews. + Provide education to members on the telephone listening for potential opportunities for improvement in drug regimen. + Work with prescribers to facilitate appropriate changes in drug regimen. _Immigration or work visa sponsorship will not be provided for this position_ Hiring Compensation Range: $120,000 - $145,000 annually _Compensation may vary based on factors including but not limited to skills, education, location and experience._ In addition to base compensation, associates may be eligible for a scorecard incentive, full range of benefits and generous paid time off.
