Jet Health, Inc / First Choice Home Health is seeking a dedicated full-time home health Clinical Scheduler to join our home health team. The primary responsibility of this role is to efficiently schedule patient/client services and coordinate with staff to ensure the delivery of high-quality care to our patients in Boise, ID, and surrounding areas.

This position could potentially handle a scheduling case load of 200+ patients. 

Requirements Education: High School graduate.Experience: One year's experience as a home health scheduler highly preferred.Skills: Exceptional communication skills, experience with electronic medical records (HCHB preferred), ability to multitask in a fast-paced environment.Home Care Home Base experience highly preferred but not requiredStrong MS Office experience, Excel & Word requiredWillingness to help in other office functions as neededTeam player with a can-do attitudeStrong attention to detailAbility to follow direction and retain information 

Essential Functions:

EvaluationSchedules and appropriately documents patient/client schedules in a timely manner.Participates in coordinating care with management and patient/client interdisciplinary team.Provides effective communication to patients/clients, staff members, other health care professionals, and referral sources.Processes reports timely to ensure compliance.Responds to Agency and patient/client needs in a professional and creative manner.Promotes the agency philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.Demonstrates commitment, professional growth, and competency.Provides effective communication to patient/family, team members, and other health care professionals and maintains confidentiality.Performs other duties as assigned.
