GiaCare is now accepting resumes for a civilian Clinical Social Worker – Case Manager to work full-time in anticipation for a future position at Wright Patterson AFB in Dayton, Ohio. 

Qualifications:

Degree/Education: Master's degree in social work (MSW). Graduate from a school of Social Work fully accredited by the CSWE.

Certifications: Basic Life Support (BLS) Certification as a Certified Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers or meet the minimum experience

Experience: A minimum of three (3) year’s experience within the last five (5) years as a Social Worker-Case Manager, performing case management or discharge planning. Full time employment as a Social Worker within the last 36 months is mandatory.

Licensure/Registration: Current, full, active and unrestricted license to practice as a Licensed Clinical Social Worker.

Job Duties:

Use clinical assessment skills to identify sponsors whose family members have special medical and/or educational needs in a timely manner. Shall use a multi-disciplinary and collaborative approach with other key service providers, such as the installation youth and childcare facilities, and officer and enlisted spouses’ groups, to ensure effective outreach and identification of special needs. Ensure active-duty sponsors with family members who are identified for special needs are then enrolled into the Exceptional Family Member Program when appropriate. Provides oversight for base-level data entry in AF- provided data management systems used in the management of EFMP-M.Coordinate and participates in interdisciplinary team meetings, designated facility meetings, and care coordination meetings. Shares knowledge and experiences gained from own clinical practice and education relevant to case management.Participate in the orientation and training of other staff. May serve on committees, work groups, and task forces at the facility.
