Clinical Strategy and Practice Lead (Florida Division)
**Become a part of our caring community and help us put health first** Description: The Clinical Strategy and Practice Lead directly supports the Director of Clinical Strategy and Practice in building strategies for development, engagement, best clinical practices, and processes for the care integration teams. This role is critical in supporting the Director in building partnership with clinical-operational divisional dyads (Division CMO/Division Ops Leader) on care integration team foundational programs and future strategic opportunities for managing populations and coordinating care to reduce acute and post-acute care utilization. The Clinical Strategy and Practice Lead role is hybrid with a requirement to reside in the state of Florida to facilitate and liaise relationships between Florida market dyad leaders (i.e., Market CMO, Market President, RMDs) and Florida CIT market managers and supervisors. **Responsibilities:** + Assist the Director in implementing clinical program strategies by coordinating cross-functional teams, ensuring alignment with organizational goals, and monitoring progress against short-term and long-term objectives. + Manage day-to-day operations of the CIT program, ensuring that processes are efficient and effective. Support Director in effort to standardize workflows and optimize operational costs in line with national benchmarks. + Track and report on key operational metrics, including CIT program impact on patient care utilization and operational efficiency. Provide regular updates to the Director, highlighting areas of success and opportunities for improvement. + Assist Director with performance management activities such as but not limited to delivery of coaching, feedback, performance evaluations. + Serve as a key point of contact for market-level leaders and clinicians, ensuring their feedback is integrated into program initiatives. Support the Director in maintaining strong relationships with stakeholders, creating forums for communication, and promoting a unified approach to decision-making. + Serve as proxy as appropriate for Director in key clinical and strategy meetings, initiatives, programs and serve as lead on communication, implementation and execution of program as determined by the Director. + Assist the Director in development of managers and staff, fostering a collaborative environment. Empower team members to achieve departmental goals, focusing on associate engagement and retention, while maintaining a positive and inclusive workplace culture. + Facilitate collaboration among interdisciplinary teams to develop and execute comprehensive interventions for transitions of care and high-risk patient management programs. Ensure effective coordination with internal and external providers, including hospitals and community-based organizations. **Required Qualifications:** + Resides in the state of Florida + A current unrestricted license in state of Florida, either RN or Social Work, or related healthcare field. + CM Experience with Senior population. Experience in Value Based Care preferred. + Experience leading and developing clinical leaders and teams + Demonstrated ability to identify admin cost savings + Demonstrated strong critical thinking and strategic thought process + A highly credible leader with deep understanding and empathy for the clinician and patient pain points in day-to-day panel management. + Excellent communication skills, including follow-through communication, executive presentations, and the ability to negotiate with horizontal and upward/downward vertical networks. + A passionate advocate for improving clinician and patient experience and effectiveness through panel management (aka population health management) + Comfortable managing and supporting the Director to influence in the context of multiple operations, development, and deployment initiatives in a matrixed environment. + Strong analytical and problem-solving skills with the ability to develop and implement metrics to measure the effectiveness of care transitions. + Ability to multi-task in a fast-paced work environment + Understanding of quality and value drivers in full risk care delivery, ideally in Medicare/seniors **Use your skills to make an impact** **Scheduled Weekly Hours** 40 **Pay Range** The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $112,400 - $154,900 per year This job is eligible for a bonus incentive plan.
