Completes psychosocial assessments, treatment plans, and ongoing treatment with individuals, groups, and families in cooperation with patient and multidisciplinary team. Consults and coordinates care of patient with physician, providers, and community agencies. Delivers behavioral health care in accordance with patient evaluation, treatment plan, and within the scope of their license. Maintains pertinent patient data in medical record in accordance with state, Joint Commission, and Mercy Health System guidelines. Performs other duties as assigned. Complies with all Mercy Health System, State, and Joint Commission policies and regulations. Must be willing to travel between treatment sites, on occasion, when asked to do so during situations where program coverage is required, meetings, and/or system in-services are required. Responsibilities Essential Duties and Responsibilities Interviews clients, reviews records, and confers with other professionals to evaluate condition of client. Formulates program for prevention, treatment and rehabilitation of client. Counsels clients individually and/or in group sessions to assist client with their mental health issues and/or in overcoming alcohol and drug dependence through monitoring the client (s) treatment success and adapting treatment as needed. Counsels family members to assist family in dealing with and providing support for client. Refers client to other support services, as needed, such as medical evaluation and treatment, social services, and employment services. Prepares/maintains current reports/records. Education and Experience Master's Degree in Counseling, Social Work, Psychology or other related field. Certification and Licensure Licensed to independently provide both mental health and substance abuse treatment in Wisconsin. Mental health license includes LCSW, LPC, LMFT. Substance abuse license includes Clinical Substance Abuse Counselor (CSAC); Substance Abuse Counselor (SAC); or LCSW, LPC, LMFT with attached Substance Use Disorder specialty. Skills and Abilities Applicant must possess a broad background of training and experience with various patient populations and modes of service delivery. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills To perform this job successfully, an individual should have knowledge of preferred Database software; Internet software and Word Processing software.
