DaVita is seeking an experienced and compassionate Clinical Transplant Nurse Manager to lead a team dedicated to making a meaningful impact in the lives of patients with end-stage kidney disease. The Clinical Transplant Nurse Manager role centers on providing leadership for a team of six transplant liaisons, coordinating patient care, managing partnerships with transplant stakeholders, and driving quality improvement initiatives that support the program's mission. With a strong focus on health equity, this position is crucial in ensuring that all patients, regardless of background, receive the support needed to navigate the transplant process, from evaluation through wait listing and transplantation. The Clinical Transplant Nurse Manager will oversee and optimize daily operations within the Transplant Health Equity Learning Lab. The Clinical Transplant Nurse Manager role is responsible for managing resources, fostering a high-performing, patient-centered team, and coordinating the clinical care of transplant patients. Key objectives include improving waitlist management, increasing the transplant rate with deceased and living donors, and promoting equitable access to transplant resources. By addressing social drivers of health (SDOH), this role reduces disparities and simplifies the transplant process for underserved populations.
ESSENTIAL DUTIES & RESPONSIBILITIES
Team Leadership & Development:
Lead and supervise six Transplant Liaisons (TLs), providing clinical guidance and support to optimize their effectiveness.Develop and implement training programs to enhance TLs’ capabilities in patient advocacy, community engagement, care coordination, stakeholder engagement, and transplant outcomes.Promote a collaborative, patient-centered culture that aligns with DaVita’s mission and values.Core Competencies:
Clinical Expertise: Extensive knowledge of end-stage-kidney disease (ESKD) management, transplant processes, and patient care coordination.Patient-Centered Approach: Strong commitment to empathetic, equitable care that respects patients’ diverse backgrounds and unique needs.Communication & Advocacy: Excellent interpersonal skills to effectively communicate with patients, healthcare professionals, transplant centers, and community organizations.Leadership & Team Development: Proven experience in managing clinical teams, with a focus on motivating, training, and developing staff.Quality Improvement Mindset: Ability to identify and implement process improvements that enhance patient care outcomes and reduce disparities in transplant access.Patient Care Coordination:
Oversee transplant evaluations, waitlist management, and transplant stakeholder engagement, ensuring seamless care transitions.Act as the primary clinical resource and point of contact for complex patient cases, collaborating closely with transplant surgeons, transplant center clinical team, social workers, DaVita’s Care teams, and external providers.Facilitate communication between stakeholders, TLs, and external providers to streamline patient care and address barriers to achieving transplant outcomes.Partnership & Stakeholder Management:
Build strong partnerships with DaVita Care teams, transplant centers, healthcare providers, and community resources.Serve as a liaison between the healthcare system and Transplant Liaison to gain access to necessary resources that facilitate outcomes and access to care.Work with community organizations to address barriers such as socioeconomic factors that may impact timely transplant evaluation and waitlisting.Quality Improvement & Program Metrics:
Establish and monitor core metrics to drive improvements in waitlist management and transplant ratesIdentify areas for process improvement and make procedural recommendations to achieve team and program goals.Engage in quality improvement initiatives that address SDOH and health disparities, contributing to DaVita’s goal of being the Transplant Partner of Choice for patients receiving ESKD treatment.Assist in the collection and analysis of data as requested by DaVita, patients, and local transplant partners.

NJ, MD, PA, GA, SC, NCSPECIALIZED EXPERIENCE, EDUCATION, TRAINING, OR QUALIFICATIONS
RN, BSN, or MSN Nursing degree from an accredited college or university required.1-3 years of transplant experience requiredUnderstanding of transplant programs process and workflows required.Experience engaging in initiatives that promote health equity and social justice within program communities that improve health outcomes and advocate for underrepresented and vulnerable groups within the community required.Management in HealthCare Navigation preferredExperience in Leading Teams that address barriers to transplant in underserved populations preferred.Proficient in transplant and living kidney donation regulations and transplant center requirements, demonstrating the ability to ensure compliance. Possesses the ability to track and facilitate patient progress throughout the transplant process, from referral, evaluation, and wait listing in collaboration with local transplant partners to reach outcomes required.Proficient in health equity and social justice within the pilot communities, who performed advocacy work, community outreach, policy development, and research, eliminating health equity disparities required.CERTIFICATES, LICENSES, AND REGISTRATIONS
