Why You'll Love This Role:Community Impact: Make a real difference by providing comprehensive clinical services to a diverse client base.Leadership Opportunity: Lead a team of dedicated professionals and shape the future of our clinic.Flexible Schedule: Enjoy a work-life balance with a flexible schedule and the ability to make your own hours.Key Responsibilities:Clinical Services: Provide individual and family therapy through regular contact and home visits.Record Keeping: Maintain meticulous client records and produce necessary reports for third-party billing and licensing/accreditation requirements.Client Engagement: Maintain regular contact with clients and their families, supporting their needs through treatment planning, team meetings, and psychiatric reviews.Treatment Planning: Develop and implement individualized treatment plans (ITP) with clients, families, and collaterals.Progress Monitoring: Monitor and assess client progress, facilitating treatment team meetings and case reviews as needed.Crisis Intervention: Provide crisis intervention in collaboration with team members and supervisors.Professional Development: Attend and participate in trainings and meetings to maintain and develop professional competency.On-Call Coverage: Provide on-call after-hours coverage as part of a rotating schedule.Essential Skills:Case Management: Expertise in managing client cases and coordinating care.EMR Proficiency: Skilled in using electronic medical records.Care Management: Strong experience in managed care and pediatrics.Mental Health: Comfortable working with children and families, particularly those involved with DCF.Home Visits: Experience conducting home visits with families and children.Education: Master’s Degree required, eligible for LICSW, LHMC, or other clinical licenses.Trauma-Informed Care: Familiarity with trauma-informed care practices.At-Risk Populations: Experience working with at-risk populations.Human Development: Knowledgeable in mental health and human development.Work Environment:Office and Community: Work in a typical office setting and travel to client homes and community settings.Caseload: Manage a caseload of 8-12 cases per week, with clients aged 2 to 22 years old.Flexible Hours: Monday to Friday, with flexibility between 8am-8pm. Must work at least 28 hours per week to receive a base salary.Office Presence: Be in the office at least two days a week with the other days consisting of travel for caseload.Join us and lead a team that is passionate about making a difference in the community.
