Description and Responsibilities

The Program of Assertive Community Treatment (PACT) program provides individualized, 24/7 care and continuity of care (open-ended, lifelong) to any person who needs it within the community. The program is an alternative to traditional outpatient treatment that was designed to mimic hospital-level wraparound support to help individuals maintain in the community, rather than bounce back and forth from the hospital. Our goal is to help people retain their independence in the community and help them live a life that is not defined by their illness. The outreach is assertive, and the team is highly integrated, multidisciplinary and persists in the face of failure. We meet individuals where they are at and bring our support to them by going to settings and spaces where they feel safe and comfortable.

 

Open Sky is seeking a motivated, teamwork-oriented Clinician to join our team! The Clinician will work alongside a dedicated, clinically sophisticated workforce and have opportunities to build clinical best practices and be a part of a long-term team utilizing the principles of Wraparound and other evidence-based practices. This is a full-time position (40 hours), Monday - Friday, with one required evening and rotating weekend coverage.

 

Key Responsibilities Include:

Screen, assess, and reassess individuals served for substance use, addiction risk, and addiction treatment needs.Lead individual therapy and family therapy. Develop and facilitate evidenced based treatment groups.Develop, coordinate, and deliver trainings in conjunction with the ACT team members to address the clinical learning needs of staff (crisis intervention, developing a clinical perspective, co-occurring disorders, etc.).Participate in on-call rotation and rotating weekend coverage. Qualifications

Master’s Degree in Social Services field, experience working with adults with mental health challenges, a valid driver’s license, and acceptable driving record required.

Pathway to LMHC/LMFT

$63,000- $64,000

LCSW

$67,000- $68,000

Independent License (LICSW, LMHC, LMFT, LADC I)
