Sage House is a licensed 16-bed mental health rehabilitation center (MHRC) serving San Mateo County residents, 18-59, with long histories of mental illness and multiple episodes of acute psychiatric hospitalization.
Shifts: Short-Hour | Hours: 8:00 am-4:30 pm | Days: Sunday - Monday
Expected starting wage is $34.62. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
POSITION OBJECTIVE
The Social Worker provides general casework services to assigned clients and their families. This involves coordinating and collaborating with other services and agencies such as the Conservator's Office, Public Defender's Office, Child Protective Services, Probation Department and County Mental Health Services.
QUALIFICATIONS
One (1) year of direct service experience with mentally ill individuals in an inpatient or outpatient setting is required. A Masters Degree in Social Work from an accredited graduate school of Social Work and a valid California Driver's License are also required. Necessary skills include the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding of the conservatorship process and knowledge of basic medical and psychiatric terminology. Criminal justice clearance and education verification are required.

Provides consultation to clients and their families regarding issues arising from the clients' admission, discharge and/or aftercare plan. Participates as a team member and provides input via reporting observations, concerns and asking appropriate questions. Reviews and implements resident treatment plans. Actively facilitates and participates in multidisciplinary team meetings and treatment planning meetings. Demonstrates knowledge of Professional Assault Response Training (PART) principles and uses PART concepts to prevent and safely manage aggressive behavior. Provides leadership in facilitating the team process. Provides education/training to other team members as necessary.ASSESSMENT
