Summary :The Coder (Hospital - II) reviews patient medical record documentation and assigns and sequences ICD-10-CM diagnosis codes and ICD 10 procedure codes, when applicable, to inpatient and Children Hospital of Wisconsin IP/OP/OBS accounts, abiding by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and in compliance with official coding guidelines and other regulatory requirements. Communicates with providers when needed to obtain complete and specific documentation to accurately assign codes. Follows up and obtains clarification on documentation issues. Maintains department standards for timely coding completion.

Job Description:

KEY ACCOUNTABILITIES:
1. Reviews patient medical records for day surgery and observation accounts.
2. Assigns ICD-10-CM diagnosis codes and ICD 10 procedure codes, when applicable, to all accounts, and sequences as outlined in the ICD Official Coding Guidelines, Uniform Hospital Discharge Data Set (UHDDS), CPT/HCPCS Guidelines, and other regulatory requirements.

3. Communicates with providers and other clinical team members to obtain additional information, diagnoses specificity, or documentation needed to accurately assign codes.
4. Manages follow up work queues to ensure timely coding completion.

5.  Participates in ongoing education for ICD-10 updates, other educational opportunities and regulatory/compliance updates.  Keeps up-to-date with AHA Coding Clinic publications.

6.  Utilize 3M 360 encoder.

7. Works NCCI and LCD edits for Children accounts.

8. Coding IP Rehab and psych accounts specific to TCN

QUALIFICATIONS:
• High school diploma or GED
• One year of hospital or office CPT surgical coding experience.
• Diploma/certificate in coding: Preferred: CCS (Certified Coding Specialist, (AHIMA), or COC (Certified Outpatient Coder), (AAPC), or CIC (Certified Inpatient Coder), (AAPC) Or: RHIT (Registered Health Information Technologist) (AHIMA). Will accept AHIMA or AAPC certifications.

PHYSICAL DEMANDS:
• Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
• Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:
• Normally works in climate controlled office environment - this position is for remote/at home coding
• Frequent sitting with movement throughout office space
• Use of computers throughout the work day
• Frequent use of keyboard with repetitive motion of hands, wrists, and fingers

Hours can be flexible between 7-3:30pm or 8-4:30pm M-F.
