Responsibilities HIM Technician/Coder JOB SUMMARY Assembles, analyzes, abstracts, and codes medical records. Adheres to current ICD10 and DSM-V requirements and ethical coding standards Reconciles daily readiness report. Enters data into departmental computer system. Files miscellaneous reports. Retrieves records as requested. Answers department telephones. Processes release of information requests. Complies with hospital policies and procedures and state/federal regulations regarding confidentiality of records. Participates in employee orientation and on-the-job training. Participates in Performance Improvement activities – concurrent and retrospective chart reviews and other duties as assigned by the Medical Records Department Director. DEMONSRATES COMPETENCES IN THE FOLLOWING AREAS: Responsible for accurate coding of all discharged records of the hospital. Adheres to current ICD10 and DSM-V requirements and ethical coding standards. Responds to coding audits in a professional manner. Produces information regarding physician documentation requirements to improve coding practices. Collaborates with business office to ensure coding accuracy. Maintains coding certification(s) in good standing. Participates on corporate conference calls required for coders to attend. Maintains ethical coding practice. Consults physician when clarification is needed in documentation. Assists the department in processing medical records requests. Has a thorough understanding of current coding software (Integrity 360). Knowledgeable about TJC, CMS, state, and local health information requirements. Qualifications REGULATORY REQUIREMENTS: Education: High School diploma required; Associates in Medical Coding preferred. Licensure/Certification: Successful completion in an AHIMA or AAPC approved medical coding certification program. Must have RHIT certification at time of hire. Maintains coding certification(s) in good standing. Experience: 2-3 years medical records experience preferred. Minimum one year experience ICD- 10-CM procedures and use of DSM V coding procedures required; use of an encoder software product for code assignment preferred. Knowledge: Working knowledge of medical terminology, ICD-10-CM and DSM V nomenclature. Good organizational skills with the ability to prioritize in a fast-paced environment. Good written and oral communication skills. Computer literacy skills and typing skills required. Physical Demands Days/Hours: Fulltime - Days Must be reliable and punctual Must be able to exert physical requirements to lift and maneuver objects, medical records, this includes kneeling, bending, reaching, pulling, crouching, and pushing medical records. Must be able to sit for prolonged periods of time. Must have the ability to move throughout the facility independently. Must know how to gather and analyze data. Must be detail oriented and possess superior documentation skills. Must demonstrate initiative, accountability, and good judgment. Able to work diligently and accurately Must maintain a minimum 90% coding accuracy rate.
