Summary

The Office of Community Outreach, Engagement and Equity at Moffitt Cancer Center is seeking a dynamic and dedicated individual to join our team as a Community Navigator II. This is an exciting opportunity to help others in community overcome barriers that keep them from receiving timely access to cancer screening, cancer care, and resources for improved cancer outcomes and reduced health disparities.

Position Overview

The Community Navigator II provides day-to-day guidance and oversight to the linkage and navigation functions of the Office of Community Outreach, Engagement and Equity. The position will lead the Community Patient Navigator. It is expected for the role to perform work with minimal guidance and supervision. Your Primary responsibilities will include:

Administering evidence-based assessments to identify barriers that may be preventing community members from receiving cancer screening and care. Connecting community members and referred patients to health and community resources. Establishing partnerships with local community-based organizations and community providers to create strong referral networks. Conducting outreach and education in community on cancer-related topics, including navigation services, to reduce psychosocial fears and barriers related to cancer screening and care. Advocating on behalf of the community and patients to improve access to cancer screenings, treatment, and program resources. Mentoring, guiding and delegating work assignments to the Community Patient Navigator. Creating standardized reporting that is inclusive of metrics that accurately summarizes navigation activities and patient/community interactions. Minimum Qualifications

The ideal candidate for this position will possess the following qualifications:

Education: A Bachelor's degree in Social Work, Counseling, Public Health, Healthcare Administration, Nursing or other human services related field is required. Experience: A minimum of three years of experience in navigating community members and/or patients, while in healthcare, community-based or social work environment. Inclusive of the three total years is one year of project or people leading experience as a team lead, mid or large size project leader or supervisor. Skills: Strong organizational, planning, communication, and interpersonal skills. Ability to work effectively in a collaborative, interdisciplinary team environment as well as be an independent self-starter. Knowledge: Familiarity with community-based program services, public benefits, and other resources as well as how to identify, collate and use new resources is preferred. Also, understand patient care; including the role culture plays in a person’s health; and health care pathways.
