The Cognitive Restoration Case Manage is a non-exempt position paying $26-34 per hour at 35 hours per week. The hours of operation are Monday- Friday 9:00AM-5:00PM and the office is located in Kew Gardens, NY. The role reports to the Program Director. Primary Purpose of Job: EAC Network is seeking an experienced Cognitive Restoration Case Manager to join our Queens Treatment Court project team. The Cognitive Restoration Case Manager will be responsible for providing comprehensive case management and support services to individuals participating in the Queens Treatment Court program. This is a critical role in helping our clients address cognitive impairments and mental health challenges, and successfully complete the program. Principal Duties & Responsibilities: + Conduct comprehensive assessments to identify clients' cognitive, mental health, and social service needs. + Develop individualized treatment and service plans in collaboration with clients, the treatment team, and community providers. + Coordinate and monitor the delivery of cognitive restoration therapies, mental health services, and other support services. + Provide one-on-one coaching and counseling to help clients develop coping strategies and life skills. + Advocate for clients and serve as a liaison between the court, treatment providers, and community resources. + Maintain detailed case notes and documentation to track client progress and outcomes. + Participate in regular team meetings and collaborate with the broader treatment court staff. + Collect and analyze data to measure the program effectiveness and identify areas for improvement. Knowledge, Skills, & Abilities Required: + Bachelor's Degree in social work, psychology, or a related human services field required. + Experience in case management, mental health, or cognitive rehabilitation services. + Demonstrated knowledge of cognitive impairments, mental health disorders, and evidence-based treatment approaches. + Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse clients. + Excellent problem-solving, critical thinking, and organizational skills. + Proficient in using electronic health record systems and data analysis tools. + Familiarity with the criminal justice system and experience working with justice-involved populations is preferred. + Bilingual (English and Spanish) is a plus.
