Abilities First is seeking a full time Community Habilitation Support Specialist. Community Habilitation offers a wealth of valuable, one–on–one services to children and adults with disabilities who live at home with their families. The program offers choices and increased independence while encouraging productivity and shared community involvement. This entry level supervisory position will provide training and supervision to assigned staff in the delivery of services and supports to program participants in their home and in the community.
Schedule: Monday through Friday, 40 hours per week with flexibility to adjust schedule to provide weekend coverage as needed.
Hourly rate of pay: $18.10 - $20.10
Responsibilities include:
+ Provide mentoring, direct support training, and oversight to staff, during all hours of Community Habilitation services.
+ Attend ISP/Lifeplan meetings to develop Com Hab plans with a person-centered driven approach.
+ Assist Coordinator in monitoring the job performance of their designated program staff, ensuring staff demonstrate proficiency in all goals and skillsets outlined in the NYS DSP Core Competencies.
+ Maintain timely documentation as required by oversight agency and service program.
+ Responsible for lifting, transferring, physically assisting and maneuvering participants of all ages.
+ Assume driving responsibilities as needed.
+ Responsible for carrying emergency phone coverage as per assigned schedule.
+ Other appropriate job-related activities as requested by your supervisor or as circumstances warrant.

Requirements
+ High School or Equivalency Diploma required and minimum 1 year of experience working with persons with developmental disabilities, including experience with individuals who have behavioral challenges required. A combination of education and experience which would be considered by the employer as appropriate to qualify for the position may be acceptable.
+ Supervisory experience preferred.
+ Basic computer skills are required.
+ Must have a current NYS driver’s license deemed “acceptable” by our insurance carrier.
+ Possess verbal and written communication skills in English to ensure adequate regulatory documentation.
+ Have the ability to stand and walk for long periods of time
+ Have the ability to push and pull individuals in wheelchairs, lift, bend, squat, turn in the knees, pivot, and grasp with both hands (in order to properly and safely perform the techniques taught in CPR/First Aid, behavior intervention techniques, and lifting and carrying techniques), with or without a reasonable accommodation.
