Job Summary:
Join our compassionate and collaborative team as a Community Behavioral Health Specialist, where you will play a vital role in helping individuals achieve and sustain recovery from mental illness and substance use disorders. As a Community Behavioral Health Specialist, you will complete initial and annual assessments, develop person-centered wellness plans, and facilitate educational groups. You will also act as a liaison to community organizations, assisting clients in accessing necessary resources to support their recovery goals. Your role is essential in providing crisis services and case management to meet the needs of our clients daily.

Complete initial and annual assessments within the electronic medical record (EMR) for individuals entering ACS services.Develop person-centered and strengths-based wellness plans.Provide educational information and skill-building to clients and families regarding daily living skills, education, symptom management, and communication to improve functioning and quality of life.Act as a liaison to community and service organizations, assisting clients in gaining access to necessary wellness and rehabilitative services.Facilitate educational groups utilizing a motivational approach as assigned.Complete all required documentation in the EHR and for the Department of Mental Health.Participate as an integral part of the ACS team, providing crisis services and case management as needed.Education, Experience, and/or Credential Qualifications:
Master’s degree in Counseling or Social Work required.Minimum of two years of experience working with adults with Serious and Persistent Mental Illness (SPMI) or children with Serious Emotional Disturbance (SED).Must acquire and maintain required training per agency policy and DMH regulations, including documented attendance of professional development/training or education.Additional Qualifications:
Current driver’s license, acceptable driving record, and current auto insurance.Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.Physical Requirements:
Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts.
