As a Community Care Registered Nurse (RN), you’ll provide both clinical and psychosocial care to individuals experiencing homelessness in community settings. Primary responsibilities include:

• Providing care to individuals experiencing homelessness at locations such as encampments, interim housing/shelter sites, community health and resource fairs, and on the streets.

• Conducting patient assessments and documenting data through interviews, reviews of medical records, and analysis of diagnostic test results.

• Collaborating with providers, social workers, and other health professionals to develop effective care plans.

• Sharing responsibility for driving the Community Care Van to various service locations.

Additional Schedule Information

Shifts may start as early as 7:00 a.m. or as late as 1:30 p.m., with typical shift hours as follows:

• 07:00 a.m. – 03:30 p.m.

• 08:00 a.m. – 04:30 p.m.

• 09:00 a.m. – 05:30 p.m.

• 10:00 a.m. – 06:30 p.m. 

• 11:00 a.m. – 07:30 p.m.

• 01:30 p.m. – 10:00 p.m.

Because we provide patient care seven days a week, all RNs work at least two weekend shifts per month. Schedules are provided in advance.

Salary Range: $59.46/hour – $88.39/hour
