Then our Community Care Specialist position is for you! New Day Healthcare is currently seeking a Community Care Specialist in the Wichita, Ks area. Pay rate: $18-20 per hr Full-time-Monday-Friday 8-5 p.m. Work from home or in the office! Responsibilities: + Receive incoming calls and messages from potential clients and consistently strive for emotional linkage and developing relationships. + Determine client needs and assist in coordinating care by referring to appropriate service lines. + Collect, share, and manage information using respective databases. + Clearly communicate detailed information to appropriate teams. + Track referrals, keep status and notes up to date in database, and advocate for patient care. + Internally build relationships for the furthering of serving clients. + Make outgoing calls to patients in the care continuum. + Performs other related duties as directed by the Supervisor. Qualifications, skills and abilities: + Patient, attentive and empathetic listener. + Must have strong phone etiquette with the ability to communicate well, both verbally and in writing. + Must be detailed-oriented, accurate, organized, multi-tasker, and great with time-management. + Initiative-driven individual able to work independently and in a group setting in a fast-paced environment. + Computer proficiency and knowledge of Microsoft products. + Must be reliable, self-starter, strong work ethic and work well within a team. + Previous sales or marketing experience preferred. + Experience in healthcare preferred. + Will train the right candidate.
